Hospice Social Workers

Social workers are offered to every patient to help with social services needs such as insurance questions, funeral planning, and psycho-social welfare dilemmas.

Hospice care is about much more than just receiving medical care. In hospice, a devoted and compassionate team of doctors, nurses, home health aides, therapists, counselors, volunteers and social workers provide a deep, supportive and nurturing environment that allows them to deliver the absolute pinnacle of care and support to each and every patient.

Depending on the needs of each patient, the hospice care team can also help coordinate care, such as home visits and other community services based on the individual needs of the patient.
